Senior Living Liaisons values its user's privacy. Accordingly, we have developed this policy in order for you to understand how we collect, use, communicate, disclose and make use of personal information. The following outlines our privacy policy:
  • Before or at the time of collecting personal information, we will identify the purposes for which information is being collected and inform our clients of such purposes.
  • We will collect and use of personal information solely with the objective of fulfilling those purposes specified by us and for other compatible purposes, unless we obtain the consent of the individual concerned or as required by law.
  • We will only retain personal health and financial information as long as necessary for the fulfillment of the referral purposes.
  • We will collect personal information by lawful and fair means and, where appropriate, with the knowledge or consent of the individual concerned.
  • The personal data we collect is relevant to the purposes for which it is to be used and, we prioritize that collected data is accurate, complete, and up-to date.
  • We will protect personal information by reasonable security safeguards against loss or theft, as well as unauthorized access, disclosure, copying, use or modification.
  • We will make readily available to customers information about our policies and practices relating to the management of personal information.
  • A disclosure and advisory form will be provided to our clients. Written consent is obtained, and no personal information is shared with prospective placement settings without prior consent. Signed disclosure and advisory forms are retained for 3 years.
  • Personal health and financial information is shared only with facilities to which the client is being referred and is not maintained long-term beyond what is necessary for referral purposes.
  • Our clients can request access to their personal information, request corrections, withdraw consent, and submit privacy related questions or concerns by contacting their agent via phone or in writing via email. The client will receive verbal or written response to their requests by their agent within 3-5 business days.
We are committed to conducting our business in accordance with these principles in order to ensure that the confidentiality of personal information is protected and maintained.
